From time to time the GPs are asked to provide a service which is not covered by the NHS. Items of work such as these will be charged for and list of charges is set out below:

Medicals

• HGV, Taxi, Ofsted, Fostering, Adoption – £95
• To whom it may concern letters – £20
• Holiday cancellation form completion – £25
• Fitness to travel certification – £25
• Sickness or Accident Insurance claim form completion – £25
• Private Sick Notes – £20
• Power of Attorney Examination – £95
• Home Visit Examination – £120
• Report (No Examination) – £65
• Occupational Health Report – £50

Chargeable Vaccinations

• Hepatitis B (Occupational Health reasons refer to protocol) – For travel £40 per dose
• Mengivac ACWY – £85
• Rabies – Booster £70, Course of 3 £175
• Japanese Encephalitis – Booster £95, Course of 2 £190

All payments must be cash or cheques only and made payable to port view surgery. Vaccinations must be paid for before they are ordered.
