Care Navigation/ Signposting
Our team of Patient Advisors’ are trained to direct you to the best and most appropriate course of treatment for your condition. Many problems do not need to be dealt with by a GP and in fact rather than waiting for a GP appointment you will often be treated more quickly by seeking help from other clinicians within our team, or another service based elsewhere.
Our team at Port View now consists of:
- An Advanced Care Paramedic – Kirstie is highly trained in Acute medicine and is able to provide on the day assessment and care, as well as managing on going complex care for our patients.
- Two First Contact Physios – Paul and Philippa are your first point for any muscular skeletal issues. They can order tests such as x-rays and can refer you onwards for treatment if necessary – preventing you needing to see a GP at all.
- A Clinical Pharmacist- Alice is able to provide help and advice regarding all aspects of your medication, she also conducts routine and more complex medication reviews.
- A Social Prescriber – Mandy can help with social issues which might be affecting your health, loneliness, benefits issues, housing to name a few and is able to help you find the help and resources you need
Be prepared to be signposted to another member of our team when seeking a GP appointment, we will be providing you with the quickest and most appropriate avenue to access the care you need.
